Workers' Compensation

All employees are covered by Workers’ Compensation insurance, which may compensate employees for lost time, medical expenses, and loss of life or dismemberment from an injury arising out of or in the course of work. If an employee is injured at work, they are required to report the incident to their supervisor. The report should be made as soon as possible, but no later than 24 business hours. An injured employee may need care beyond what can be provided at the worksite. If this is the case, the employee should visit one of the approved Designated Occupational Health Care Providers listed below.

An injured employee may need care beyond what can be provided at the worksite. If this is the case, the employee should visit one of the approved Designated Occupational Health Care Providers.

Physician Authorization - Return to work letter and worksheet - This letter and form may be sent with an employee to their treating Workers' Compensation doctor to document any temporary work restrictions they deem necessary for the employee. The employee's job description may also be provided to the doctor to assist in accurately documenting applicable restrictions.

Please use this form to report ALL employee work injuries. If the employee needs medical care beyond what can be provided at the site, Human Resources will use the information provided in this form to complete the FROI and submit it to the State Insurance Fund. 

If an employee doesn’t seek medical care at the time the Report of Work Injury form is completed but later decides to seek medical care, please email benefits@boiseschools.org to notify HR that a FROI needs to be completed. Include the employee’s name, the original date of injury, and which of the Designated Occupational Health Care Providers the employee visits. 

If an injured employee receives medical care beyond what can be provided at the site, they will need to meet with their supervisor prior to returning to work to complete the Work Comp Status Update form. This form should be completed each time the employee is re-evaluated by a healthcare provider for their work injury including when they are released to full duty.

This report is to be completed within ONE WEEK following each accident resulting in an injury to the following: 

STUDENT

(1) Severe enough to cause the loss of one-half day or more of school time, and/or 

(2) Severe enough to require a doctor's treatment, and/or 

(3) Severe enough to cause the loss of one-half day or more of activity during non-school time. 

NON-EMPLOYEE ADULT: Severe enough to require a doctor's treatment.
