The Sick Leave Bank is designed to help employees avoid financial hardship when they are unable to work due to illness or injury. If an employee uses all of their available paid leave, they may apply for a Sick Leave Bank grant to continue receiving pay. The district does not provide a separate short-term disability benefit. All Sick Leave Bank grant requests are reviewed and approved by the appropriate Sick Leave Bank Committee.
To opt out of the Sick Leave Bank, print and sign this form and bring it to your onboarding appointment with Human Resources. If you do not complete this form and submit it to Human Resources during onboarding, you will automatically be enrolled in the Sick Leave Bank.

Membership:
Unless an employee opts out of the Sick Leave Bank during onboarding, they will automatically contribute the number of days required, as established by the Sick Leave Bank Committee, and become a member of the Certified Sick Leave Bank. Contributing one day of sick leave is currently required for membership.
Grants:
Sick Leave Bank grants are only for the medical needs of an employee. Grant requests will be reviewed by the Certified Sick Leave Bank Committee once a month. In order for a certified employee to be eligible to apply for a Sick Leave Bank grant, the employee must meet the following criteria:
Be a member of the bank, and
Have been absent from work due to disability resulting from illness or injury, and
Have used all of their accumulated sick and personal leave, and
Pay the cost of a sub for five days where their salary was reduced by substitute’s pay at the rate of $110 per day (Chargeable Leave at the Sub Rate of Pay).
Forms:
HIPAA Authorization for Release of Information (This release is required in order to share protected health information with the Sick Leave Bank Committee to determine whether or not to grant a request).
