Willamette Dental is both your dental insurance provider and dental care provider—offering a seamless experience by delivering services directly at their clinics. With locations in Boise, Meridian, and Nampa, members receive comprehensive care with no annual maximum for most services and no deductible, making it easy to access high-quality dental care with predictable, affordable copays. 

Plan Year & Enrollment

The benefit plan year starts September 1st and ends August 31st. Once you have enrolled in the plan, the plan will automatically renew each year unless you make allowable changes during the Spring Enrollment period or you experience a qualifying event allowed by the IRS for Cafeteria Plans. For more information, visit the Benefits webpage. The deductible will reset at the beginning of each plan year.

Insurance Cards

Once enrolled in the plan, employees will not receive a membership card. The provider will be able to look up the employee by the social security number.

Eligibility

To be eligible for coverage, dependents must meet the eligibility requirements:

1. The legal spouse of the member is eligible for coverage as a dependent.

2. A child is eligible for coverage as a dependent to age 26. Children of retired members (and the retired member’s spouse) must have been previously covered as a dependent while the member was an active employee to enroll under the retiree plan.

3. An unmarried child is eligible for coverage as a dependent beyond the limiting age if all of the following conditions are met:

   1. The child is and continues to be incapable of self-sustaining employment because of an intellectual disability or physical disability and who became so incapable prior to attainment of the limiting age.

   2. The child is and continues to be chiefly dependent upon the member (or member’s spouse) for support and maintenance.

   3. Proof of the Child’s incapacity is provided within 31 days after the Child’s attainment of the limiting age. Proof may be requested annually.
