Biometric Screenings: Your First Step Toward Wellness Savings
Biometric screenings are quick, confidential health assessments that measure key health indicators such as blood pressure, cholesterol, glucose levels, and body composition. These screenings are the only requirement for eligible Boise School District employees and retirees under age 65 to opt into the Regence Wellness Medical Plan, which offers lower premiums, copays, and deductibles.
Participation in a biometric screening helps you stay informed about your health while opening the door to valuable medical plan savings—without sharing your results with the District.

Simple, Standard, and Confidential
Biometric screenings are quick health assessments designed to give you a snapshot of your overall wellness. They’re performed by trained professionals and take only about 15 minutes to complete. These screenings are non-invasive and are focused on key indicators of health:
Blood Pressure – Helps detect signs of elevated stress or cardiovascular risk
Blood Draw – Measures cholesterol, glucose, and other important health markers
Waist Circumference – Assesses central risk factors
Height & Weight – Used to calculate Body Mass Index (BMI)
Tobacco Use – Captures self-reported usage
These standard screenings are confidential and results are not shared with the District. They are provided for your personal awareness and serve as the only requirement for being eligible to opt into the Regence Wellness Medical Plan.
Eligibility & How to Opt In to the Wellness Medical Plan
Who is Eligible?
Active Boise School District employees enrolled in the District’s medical plan
Retirees under age 65 enrolled in the District’s medical plan
Dependents are not eligible to participate in the biometric screening
How to Opt In to the Wellness Medical Plan:
Complete and submit a biometric screening prior to March 31, 2026.

At a District-sponsored screening location
With your own healthcare provider
Unable to Participate? - Submit a Biometric Screening Exemption Form by March 31, 2026 to remain eligible for the Wellness Medical Plan. Please note: A medical provider is required to complete a portion of the exemption form.
Only the primary medical plan member is eligible to complete the screening
All screenings must be completed and submitted by March 31, 2026 to qualify for the 2026–27 Wellness Medical Plan
🧪 Reminder: Screening results are confidential, not shared with the District, and do not impact your eligibility based on health outcomes—only completion is required.
📅 Next Step: Employees who complete and submit the biometric screening by the March 31, 2026 deadline are able to select (opt into) the Wellness Medical Plan during Open Enrollment in Spring 2026.

The Biometric Screening Verification Form for the 2025–2026 Wellness Program is now available.
If you prefer to complete your screening with your own healthcare provider instead of waiting for a District-hosted clinic this fall, you can now download the form and plan ahead.
Biometric screenings help identify health risks early and are required to be eligible for the Wellness Medical Plan.
Results are confidential and will not impact plan eligibility.
Only the primary medical plan member needs to complete the screening to opt-in.
You are completing your screening during the 2025–2026 plan year to opt-in for the 2026–2027 Wellness Medical Plan.
Forms accepted starting: September 1, 2025
Final submission deadline: March 31, 2026
If a biometric screening is medically inadvisable, an Exemption Form may be submitted by March 31, 2026 to maintain eligibility.
