Workers’ Compensation Information
All Boise School District employees are covered by Workers’ Compensation insurance. This coverage may provide benefits for lost wages, medical expenses, or, in serious cases, loss of life or dismemberment resulting from a work-related injury or illness.
If an employee is injured on the job, they must report the incident to their supervisor as soon as possible—no later than 24 business hours after the injury occurs.
When medical attention is needed beyond what can be provided at the worksite, employees should seek care from one of the approved Designated Occupational Health Care Providers listed below.

Please use the form link below to report all employee work-related injuries. If medical care beyond what can be provided on-site is needed, Human Resources will use the information from this form to complete the First Report of Injury (FROI) and submit it to the State Insurance Fund.
If an employee does not seek medical care at the time the injury is reported but chooses to do so later, please notify Human Resources by emailing benefits@boiseschools.org. Be sure to include:
The employee’s full name
The original date of injury
The name of the Designated Occupational Health Care Provider the employee is visiting
If an injured employee requires medical care beyond what can be provided at the worksite, they should visit a Designated Occupational Health Care Provider approved by the District.
Note: Most provider offices have their own Return to Work or Medical Report Update forms.

A Return to Work letter and worksheet to help the physician document any temporary work restrictions
The employee’s job description to assist the provider in aligning restrictions with job duties
If an employee receives medical care beyond what can be provided at the worksite, they must meet with their supervisor before returning to work to complete the Work Comp Status Update form.
Important Reminders:
This form should be updated after each medical evaluation, including:
Initial treatment
Any follow-up visits
When the employee is released to full duty
The completed form must be forwarded to Benefits at benefits@boiseschools.org
Why It Matters:
Accurate and timely documentation ensures:
The employee’s work status and any restrictions are clearly communicated
You can coordinate safe and appropriate work assignments
Compliance with Workers' Compensation regulations
If an employee is injured outside of work and has medical restrictions that prevent them from performing all aspects of their job, they must complete a Return to Work Status Update form before resuming work.
The employee should meet with their supervisor to complete this form after each update from their healthcare provider, including when they are released to full duty.
If the employee is absent for 10 or more days due to the same condition, they are required to apply for a leave of absence.
This report must be completed within ONE WEEK of any accident resulting in injury involving a student or non-employee adult, based on the following criteria:
For Students
Submit a report if the injury is:
Severe enough to cause the loss of half a day or more of school time
Requires treatment by a doctor
Severe enough to cause the loss of half a day or more of activities during non-school time
For Non-Employee Adults
Submit a report if the injury:
Requires treatment by a doctor
